Bible Question (short): Explain the Old vs. the New Covenant?

Question (full): Please discuss the Old Covenant, the New Covenant and the transition from one to the other. Specifically I am interested in who the Old Covenant was directed at? What was its purpose? How/could someone fufill the Old Covenant? Why was there a need for a New Covenant? Any scriptural references would be great! Thanks.
